The medal was conferred upon Bulgarian mothers in recognition of bearing and raising three or four children.
It was originally awarded in two grades, I Class for four children and II Class for three children.
In 1966, the II Class was cancelled and the I Class was awarded for raising a minimum of three children.
The medal was discontinued entirely in 1991.
There are four main versions of the I Class medal.
The medals issued first are worn on a bow ribbon and have the 1947 Bulgarian coat of arms on the reverse with the inscription "9.IX.1944."
The medals issued second are worn on a bow ribbon and have the 1971 Bulgarian coat of arms on the reverse with the inscription "681 1944."
The medals issued third have an attached metal suspension that is enamelled with the colours of the Bulgarian flag, and have the 1971 Bulgarian coat of arms on the reverse with the inscription "681 1944.""
The medals issued fourth have an attached metal suspension that is enamelled with the colours of the Bulgarian flag, and have the reverse inscription "МЕДАЛ ЗА МАЙЧИНСТВО."
