Awarded for supreme acts of gallantry, limited to non-commissioned officers (NCO) after 1981.

This medal is comprised of silver with the obverse bearing the bust of the Sultan, surrounded by the Jawi inscription "SULTAN ISMAIL NASIRUDDIN SHAH NEERI TRENGGANU DARULIMAN AL-ISLAMIAH AL-MALAYUWIAH". The reverse has the State Crest with the name of the medal "KEBERANIAN HANDAL". The ribbon is black with an narrow white central stripe. Bars may be awarded for subsequent acts of gallantry.
