The Medal of the 3rd of May was established in the Republic of Poland in 1925. It was intended for annual conferral on May 3 in commemoration of the signing of the Polish Constitution on May 3, 1791, but it was only ever awarded in 1925.

It was awarded to Polish citizens in recognition of outstanding contributions to the Polish military and social life.

A total of 4675 Silver Medals were awarded. The obverse inscription translates to "Polish Republic.”
