The Medal for Outstanding Achievement in Sport was established by the Polish People's Republic in 1954 but was discontinued following the collapse of the Polish People's Republic in 1989. It was awarded to Polish citizens in recognition of achievements in sport on the national or international stage.
The II Class Decoration was awarded to athletes who had won silver at the Olympics, world championships, or European championships.
