Awarded for services rendered during the reign of the Sultan Ismail.

The breast star is added here as a precautionary measure, as there is little evidence to support that it exists as a component of the order, although it would follow a pattern set by different orders of the sultanate

Known in Malay as "Darjah Setia Sultan Ismail Johor Yang Amat di-Berkati".
